Despite nearly 100 Medicare Advantage plans available in the state of Nevada, there’s only one 5-star-rated plan available on the market in 2025, and that’s the Alignment Health Plan. Here’s what you need to know.

5-Star Medicare Service from an Alignment Health Plan

The Alignment Health Plan by Alignment Health, is a Medicare Advantage plan that has earned a 5-star rating from Centers for Medicare & Medicaid in 2025.

The federal government (the Centers for Medicare and Medicaid Services, also known as CMS) provides an annual rating to Medicare Advantage plans based on criteria such as quality of care, customer service, and member experience. Each plan receives a rating of one to five stars, with one being the lowest and five being the highest. Medicare Star Ratings can help us recognize which plans are performing well in our area. Medicare updates these ratings each fall for the following year.

With a 5-star rating, the Alignment Health Plan was recognized in Nevada as a “Best” Medicare Advantage insurer by the U.S. News & World Report.

Alignment Health Plan – award-winning features

In 2025, there are four Health Maintenance Organization (HMO) plans offered by Alignment which include chronic conditions and dual-eligible special needs plans (SNPs).

Here are some key highlights of Alignment’s Nevada plans:

  • $0 copay for Part D generic prescription drugs on its Platinum + Instacart (HMO) and smartHMO plans, which earned 5 out of 5 stars from CMS in 2025.
  • $0 premium Heart and Diabetes (HMO C-SNP) for members with a qualifying chronic condition, which offers monthly allowances for eligible over-the-counter items, groceries, gas and utilities, and home safety modifications, a 4-star rated plan in 2025.
  • The ONE/el ÚNICO (HMO D-SNP) for members enrolled in both Medicare and Medicaid, providing enhanced care coordination and additional benefits beyond what is typically offered by Medicaid or original Medicare alone.

How to join a health plan with a 5-star quality rating

If a Medicare Advantage Plan, Medicare drug plan, or Medicare Cost Plan with a 5-star rating is available in your area, you can use the 5-star Special Enrollment Period to switch from your current Medicare plan to a Medicare plan with a “5-star” quality rating. 

You can use this Special Enrollment Period only once between December 8 and November 30 the following year.

Keep in mind that if you move from a Medicare Advantage Plan that includes prescription drug coverage to a standalone Medicare drug plan, you’ll be disenrolled from your Medicare Advantage Plan, including the health benefit. You’ll be returned to Original Medicare for coverage of your health services. You can only switch to a 5-star Medicare drug plan if one is available in your area.

If you move from a Medicare Advantage Plan that has drug coverage to a 5‑star Medicare Advantage Plan that doesn’t have drug coverage, you may lose your prescription drug coverage. You’ll have to wait until your next enrollment opportunity to get drug coverage, and you may have to pay a Part D late enrollment penalty.
